The Service Facilitator is responsible for supporting individuals with disabilities to ensure the development and monitoring of the Consumer-Directed Services Plan of Care, which specifies the services the individual will receive under Medicaid. The Service Facilitator will also provide employee management training that allows the person with disabilities to properly supervise an aide. In addition, the Service Facilitators complete ongoing review activities as required by the Virginia Department of Medical Assistance Services (DMAS) or the Managed Care Organization (health insurance) for Consumer-Directed personal care and respite services.
The Service Facilitator MUST possess a minimum of an Associate Degree in a Human Service field or in Nursing. In addition, it is required the Facilitator have two years of satisfactory experience in the Human Services field working with persons with severe disabilities or the elderly. Services are to be provided in areas of Manassas, Manassas Park, Haymarket, Gainesville, and other areas of Prince William County, as assigned. Occasional night/weekend work may be necessary. Computer literacy is important. Driver’s license, auto insurance, and reliable transportation are necessary. Familiarity/personal experience with or sensitivity to people with disabilities is essential. Working at a Center for Independent Living (CIL), you will learn the services provided at the Center and be able to discuss with, and/or refer your consumers to someone else within the CIL for those services or for a referral.
